Business rates - state aid declaration

To help us to work out if you are eligible to receive a cash grant, you must declare the full amount of De Minimis aid that you have been given for the current and previous two financial years. Any assistance you have received from a public body might be De Minimis aid. This could be from central, regional, devolved governments, agencies or a local authority. 

Use this form to tell us about all De Minimis aid you have received for the current and previous two financial years. 

This form will ask you to provide

  • Your name and position within the organisation
  • Your business rates account reference
  • The full name of the ratepayer
  • Your business address and contact details
  • Correspondence address (if different from your current business address)
  • Details of all previous aid received.

This form must be completed by the ratepayer or someone authorised to do so on their behalf.
