This form will ask you to provide

  • Your details including National Insurance number.
  • The type of proof you are uploading.
  • Documents to support your application.

Types of documents you need to provide as proof to support your claim

Please note that if you have been asked to provide proof of your identity, we can't accept this electronically and you will need to bring original proof such as a passport or driving licence to one of our council offices for us to check. Photocopies are not acceptable.
