Clinical waste collection
If you need a clinical waste collection, for example infectious waste, sharps waste (such as needles), cytotoxic (or bio hazardous) waste and offensive waste you will need to contact us with a referral from your doctor. Collections can only take place once you have received confirmation from us that you have been accepted to the clinical collection list.
Use this form for all other clinical waste collection enquiries.
- Missed collection - the sharps box must be placed outside the front door by 6am on the collection day

This form will ask you to provide the following information:
- Your details
- Enquiry type
- Location where collection is required
- Reason for cancellation, if applicable
Please have this information ready as pages time-out after one hour.
